Dear Parent or Guardian,

            This fall, your school and the Weber-Morgan Health Department are partnering to provide their annual Flu Immunization Clinic. In addition to Flu vaccine WMHD will be offering the required 7th grade immunizations for students in the 6th grade (Tdap, Meningococcal and HPV). Weber-Morgan Health Department nurses will be offering immunizations to all students and school faculty. Family members are also welcome to attend and receive vaccinations at the same time. We hope this will provide an easy opportunity for students, faculty and families to receive their flu vaccine.

 Please fill out the attached Consent Form and send it with your child on the day of the Flu Clinic. We will not be able to provide a vaccine without a consent form. We will also have flu forms available during the flu clinic that you can fill out at that time. If you are unable to print the form, the school will be sending out forms with your students this week.

About Mrs. Miriam BSN, RN

Personal Life:  I am from Mexico, am fluent in Spanish and English. I have a loving toddler and pup who have filled our home with so much love and joy. I love spending time with my family, traveling, off-roading, watching movies and trying different foods!

Professional Life: I grew up and attended school in Ogden. I attended OWTC and Weber State University for Nursing. Graduated and obtained my Bachelor’s in Nursing at Weber State University. I am also a member of the National School Nurse Association and are CPR certified. I have worked in ICU and Homecare as a Registered Nurse and have loved getting to help others throughout my career. Being a school nurse has taught me so much and how to better care for student’s and their families.

My role as the School Nurse: 

My goal is to keep our students safe and healthy. I work closely with all of our great staff to help provide a safe and welcoming environment for each student. As a team, we help identify any health concerns and help provide resources to students and their families to access health care when needed.
