REGISTERING A KINDERGARTENER 

***PLEASE BRING THE FOLLOWING ITEMS TO REGISTER YOUR NEW STUDENT

-ORIGINAL BIRTH CERTIFICATE (A copy will not be accepted)

-IMMUNIZATION RECORD

-PROOF OF RESIDENCY- 2 forms of ID (Utility bill, driver’s license, lease agreement, residence disclosure. Parents name and current address must be on each form of verification.)

-REGISTRATION FORMS
-Student Information Form
-Home Language Survey
-USIIS Immunization Permission Form
-Medical Form
-Medical Examination & Dental Form (Can be turned in after the Roundup)

We are excited to have your student attend Kindergarten at Municipal and need all the above information turned in before they can start school. The sooner the better! After your students paperwork has been turned in, you will be given a student number and will need to sign up on the District Portal to finalize your registration. This will give you access to grades, attendance, lunch balances and information from the school. Please let us know if you have any questions, we are happy to help!!

Please complete the following PDF forms and email them to the Secretary Jennie Hamblin This email address is being protected from spambots. You need JavaScript enabled to view it. or print and bring them to the school                                

Please bring the following 3 forms and return to Municipal along with: 1-Immunization records  2- Original birth certificate  3- Two proofs of residency (current utility bill, updated drivers license, W2 form, etc...) as soon as you can :) 

All incoming kindergarten students will need to come in for a  pre-kindergarten assessment.  We will contact you with more information on how to sign up.
